Title V Grant – Baby Care Program

  • Grant pays a portion of salaries for nurses working in the Baby Care Program.
  • Issue identified: capturing post-partum visits is difficult if mothers see their own doctor.
  • Home health nurses (through VBDH) can encourage mothers to:
  • See their doctor.
  • Receive targeted education.
  • Concept of using an Education Prescription (Ed Rx) tool to guide and document educational outreach.


Education Prescription (Ed Rx) – Develop a mock-up for maternal health (sample sent).


We can make suggestions to enhance the content of the form. Should be able to edit, save and email the completed form.

Objectives/Goals:

Reduce mortality in post-partum period in particular.
